Release Agreement I, The parent or guardian of the player named above,being at least 18 years of age, consent to his/her participation in any baseball/softball related activities. I agree to assume all risks incidental to such participation, including transportation to and from all activities. I hereby waive, release, absolve, indemnify and agree to hold harmless Revere Youth Baseball and Softball, its officials,board members, sponsors, coaches, managers, supervisors, and persons transporting my child.
